From 31st October 2023, practices are obliged to offer patients prospective access to their medical record meaning if you have the NHS app set up you will be able to view information which is added from this point forward.
You can also access your detailed coded record if you wish by contacting the practice. If you wish to access your full record please complete the application form below and bring it with two forms of ID (one with a photo i.e. passport and one a recent proof of address) for reception to verify your identity. This form will then be passed for review before a code is posted to you. (NB due to workload there will be a delay between receiving your application and it being processed).
